I have setup add2exchange to sync calendars for 3 persons.

The sync setup is like this :

public to private : user A , B and C

private to public : user A , B and C

What happens is that if user A creates an appointment, it gets correctly synced to the public folder calendar. However it is not replicated to user B and Cs calendar on following syncs.

I have updated to the latest version to no avail. Is this setup supported?

Thanks for your complete description of the issue.  You are talking about the folder grouping model, where the folders get identical contents after synchronization.

The short answer is that you need more relationships to do that.  Items can only be copied from source to destination, not destination to another destination (said another way, you can't "chain" relationships).  So you need another relationship from the original source to your new destination.  In your case, you need one directly between each of the three users (this ends up being both ways, with each user being both a source as well as destination to each other user)
